Comments:
An unannounced monitoring inspection was conducted at the facility January 20, 2016, from approximately 9:40 am - 12:30 pm. Upon entrance there were 140 children and 19 staff. Additional staff were on site for administrative support. The Licensing Inspectors observed morning snack, reading time, bathroom procedures, hand washing procedures, diapering procedures, art, music time and letter S activites. The Licensing Inspector reviewed 4 children's records, 2 staff records and 3 medication records and 5 injury reports. The exit interview was conducted on January 20, 2016 with Program Directors.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on 1 of 4 children's records reviewed, the licensee did not ensure the records contained an immunization record before each child can attend the center. Evidence: During the inspection conducted on January 20, 2016, the records for child #4 did not have an immunization record.

Plan of Correction: The parent will be notified in person on January 20, 2016 that we need the information.

Standard #: 22VAC40-185-140-A
Description: Based on 2 of 4 children's records reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the inspection conducted on January 20, 2016, the records for child #4 did not have a physical examination.

Plan of Correction: The parent will be notified in person on January 20, 2016 that we need the information.

Standard #: 22VAC40-185-60-A
Description: Based on 1 of 4 records reviewed, the licensee did not ensure each child enrolled had a record contained with all required information. Evidence: During the inspection conducted on January 20, 2016, the records for Child #4 did not have documentation of previous child day care and schools attended by the child and proof of identity.

Plan of Correction: The parent will be notified in person on January 20, 2016 that we need the information.

Standard #: 22VAC40-185-550-M
Description: Based on 1 of 5 injury reports reviewed, the licensee did not ensure required information is documented when a child sustains an injury. Evidence: During the inspection conducted on January 20, 2016, Licensing Inspector reviewed injury report form completed on October 21, 2015, that did not document the time parent was notified of injury and injury form completed on August 25, 2015 did not have two signatures.

Plan of Correction: Corrected while inspector was present.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on 1 of 2 records reviewed, the licensee did not ensure updated background check was dated within the past 3 years. Evidence: During the inspection conducted on January 20, 2016, the record for Staff #1 had a most recent central registry check completed on November 5, 2012.

Plan of Correction: Paperwork has been given to the staff member and asked to return by Friday.
