Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, December 22, 2015 from 12:38pm to 2:34pm. The children were observed resting during their nap time. The infants were observed being held, fed and changed. they were also observed playing in bouncy chairs and exersaucers. The school aged children were leaving to go on a field trip to the bowling alley upon the inspector's arrival. Lunch consisted of potato casserole with cheese, mixed vegetables, applesauce and milk. The menu was posted. All classrooms were inspected today. Due to rainy weather the playground was not inspected and the facility?s mini buses were being used on the field trip and not available for inspection. Three children?s records and five employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill:11/10/2015 Shelter-in-place drills: 7/22/2015 & 10/22/2015 Last fire inspection: 8/20/2015 Last health inspection: 7/13/2015 Total enrollment today: 48 Today, the following child to staff ratios were observed: Infants ?6:2 Toddlers ?5:1 Twos ? 14:1 Threes and Fours?10:1 School Age ?13:2 The violations from the previous inspection were checked for corrections. There were no repeat violations found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 12/22/2015, the center did not ensure that hazardous substances were kept in a locked place. EVIDENCE: Bottles of cleaning agents were observed in an unlocked cabinet in the Toddler Room. The cabinet was out of the reach of the children. Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: The violation was corrected on site by placing the substance in a locked cabinet. The lock on the cabinet was replaced later that day so should that cabinet be used for harmful substances the cabinet would lock. Management will routinely inspect each classroom to ensure that we are continuously providing a healthy and safe environment for all children in our care.
