Standard #: 22VAC40-185-340-F Description: Based on staff interviews, the center did not ensure that children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: 1. On January 5, 2016 at approximately 5:30pm, two six year old school age children left out of the building to sneak into the van of another child's parent. Upon entering the van with their child, the parent discovered the two children and took them back into the center. 2. Attendance has been checked for the children; however, the teacher was not aware that the children had left out of the classroom, as well as the center, and thus was not within actual sight and sound supervision of the children. Plan of Correction: All staff members were re-educated on proper supervision and accountability of children within their care. All staff members understand that children must be within their sight and sound at all times. Staff must take face-to-face attendance every 30 minutes which will be documented on our company?s CSR (Child Supervision Record). Staff must also make face-to-face contact with a child when they are being dropped off and picked up from the center, to ensure proper notation of the children in their classroom. This will be reflected in the CSR in the time in/out box. Parents were notified that an incident occurred, and reminded them of the importance of escorting their child to/from the classroom and within our building, and ensuring that teacher is aware when they are leaving/picking up their child.
