An unannounced, on-site monitoring inspection was initiated and completed on 2/26/2025. The on-site inspection began at 12:35pm and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There was 1 child present who was the Provider's grandchild (1 point) and 1 caregiver with a total of 0 points. The inspector reviewed 2 children?s records and 3 caregiver/household member records on-site on 2/26/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with
applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-100-A Description: The provider is required to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Child #1 and Child #2 did not have documentation of a physical examination on file at the time of the inspection. They have been enrolled for 3 months.Plan of Correction: I will ask the parents to bring the physical exam for both children.
Standard #: 8VAC20-800-60-A Description: The provider is required to maintain an up-to-date record at the family day home for each enrolled child.
Child #1 and Child #2 were both missing documentation of proof of identity verification in their child files. They have both been enrolled for 3 months.Plan of Correction: I will ask the parents to bring proof of identity for me to verify and document.
Standard #: 8VAC20-800-90-A Description: Before a child may attend the family day home, the provider is required to obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Child #1 and Child #2 did not have documentation of immunizations on file at the time of the inspection. They have been enrolled for 3 months.Plan of Correction: I will ask the parents to bring the immunizations for both children.
