During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or

- Technical Assistance:

Discussed handwashing procedures with the Provider and best practices.
Discussed alternatives to purchasing new crib sheets to ensure a tight fitting sheet is provided for any occupied crib.
A model incident/injury form will be emailed to the provider to use as a resource.

An unannounced, on-site renewal inspection was initiated and completed on 03/04/2025, as a part of the licensure period. The on-site inspection began at 10:15a.m. and ended at 12:30p.m. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 16. The inspector reviewed 5 children?s records and 2 caregivers' and 1 household member record on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-960-E Description: Bottles shall be refrigerated and labeled with the child's full name and the date.
Bottles located in the refrigerator were not labeled with the date.Plan of Correction: All bottles will be labeled with both name and date. If the parents do not date them, staff will date them when they are dropped off.
Standard #: 8VAC20-800-320-B Description: Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
The electrical outlet in a bathroom used by children in care did not have protective covers. This outlet was accessible to children present who are under the age of five years.Plan of Correction: Covers will be placed immediately.
Standard #: 8VAC20-800-690-D Description: The diapering surface shall be cleaned and sanitized after each use.
Staff #1 did not clean and sanitize the diaper surface after changing a diaper during the inspection.Plan of Correction: The bottles of soap and water and bleach and water will be relocated closer to the changing table so it is easier to access and will be cleaned after every change.
