Comments:
An unannounced renewal inspection was conducted today, January 20, 2016 to check the facility for compliance with licensing standards. The inspector arrived at 2:00 pm and concluded the inspection at 4:00 pm. Upon the inspector arrival children were napping. Today the two classrooms were combined for nap. Soft music was playing in the background. Later children were observed having snack. Staff sat with children and converse with children in a positive manner. Children listened to a story and due to inclement weather, children watched a video rather then going out to play on the playground. The classrooms were decorated with children's art and had age and stage appropriate materials. All areas of the facility to include classrooms, bathrooms, hallways, playground and the basement were inspected. The facility has requested that the basement area be added to the usable space for the facility. The facility had the license, last inspection notice, emergency numbers, emergency evacuation procedures/maps, daily schedule and various parent information. The first aid kit and the non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Two staff records and eleven children's records were reviewed. Staff have current CPR, First Aid and daily health observation training. One staff has current MAT certification. Medication authorizations were reviewed. The most recent evacuation drill was on 12/29/2015. The two required shelter-in-place drills for 2015 were completed on 11/16/2015 and 12/29/2015. Children provide their own meals, which we labeled with their name and date. Injury reports were reviewed. Today's Ratio's: Seven children with two staff. Fire Inspection: 06/02/2015 Health 03/11/2015 The violations from the previous inspection on November 19, 2015 were checked for corrections. There were no repeat violations cited during today's inspection. A complete renewal inspection will need to be submitted before the license expiration in order for the renewal to be processed. Standard #: 22VAC40-185-140-A
Description: Based on review of children's records, the facility did not obtain documentation of the child's physical examination by a physician before the child's attendance or within one month after attendance. Evidence: The record of child #10, enrolled 09/21/2015 and child #11, enrolled 11/02/2015, did not have documentation of a physical examination in the record.

Plan of Correction: Per the Director: I will contact the parents today and request that they provide documentation of the children's most recent physical examination.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, the facility did not obtain all required information for each staff's record. Evidence: The record of staff #1, hired 11/23/2015, did not have documentation of two references of character checked before employment nor was there written documentation to demonstrate that the individual possesses the education required by the job position.

Plan of Correction: Per the Director: I will complete the two references even though it is past the date of hire. I will also request that the staff provide written documentation of her education as her position of lead teacher requires.
