Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, February 4, 2016. The on-site visit was from 10:45 a.m. to 3:00 p.m. On this date, at the time of arrival, there were fifty-seven (57) children in the care of six (6) staff members in three classrooms, two of which were balanced mixed-age groups. Three (3) additional staff members were available to assist if needed. Eight (8) children had attended the morning session of Laurel Club. There were a total of sixty-eight (68) children enrolled in the two primary classrooms, the toddler classroom and the before & after school program. The exception to compliance was noted on the Violation Notice. The risk rating for the violation was placed on the supplemental page. There were seven (7) children's records reviewed during the visit and six (6) staff members' records reviewed. The medication and medication paperwork were reviewed for three (3) children. There was an ample supply of age appropriate Montessori work activities and equipment. Circle time, Spanish lessons, Montessori work activities, singing and musical activities, transition time, outdoor play, hand washing and bathroom procedures, lunch, nap time, and dismissal procedures were observed during the visit. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of seven (7) children's records on 02/04/2016 and verification from staff, the center failed to obtain an immunization record for one child before the child's first date of attendance. Evidence: 1. The record of Child # 1 contained a record of immunizations and a statement from the physician stating the child was adequately immunized which had been obtained on 01/22/2015. This had not been received before the child's first date of attendance on 01/07/2015. 2. Staff Member # 3 verified the date the immunization record was obtained.

Plan of Correction: The center will ensure that before a child attends the school an immunization record showing that the child is adequately immunized is obtained.
