Comments:
A self-reported allegation was received in the licensing office from facility administration on 10/14/15. This investigation was initiated on 10/15/15 with a further gathering of information on 11/5/15. On 1/21/16, incident reports, accident reports, supervision and documentation in children's records were reviewed. Staff interviews were also conducted. A mandated monitoring visit was also conducted on 1/21/16, beginning at 10:45am. Children were observed eating lunch and napping during this inspection. There were 72 children present today with 8 teachers. Nine children's records, three staff records, and 4 medications with authorizations were reviewed. This inspection, including the exit interview, ended at 1:20pm. Should you have any questions, please call.

Standard #: 22VAC40-185-510-G
Description: Based on direct observation, a prescription medication for a child (#12), with parent and physician authorization, does not have an original prescription label dispensed from the pharmacy.

Plan of Correction: I have discussed this with the parents. They threw the box away. They are renewing the prescription in March and will provide us with the label from the pharmacy. Because it is an emergency medication, we will keep the medication on-site with the authorizations for the child's safety. We will remind parents of our procedures and policies for administering medications.
