Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Discussed: Changing the age of the license, modifying model forms, sanding wooden playset, online renewal system, CPR/FA in person, and the new CRS system. The inspector will send the website for the new CRS system and tips for renewal.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 3/4/2025. The on-site inspection began at 9:30am and ended at 10:58am. The inspector reviewed compliance in the areas listed above. There were 12 children present and three caregivers; the point total was 29. The inspector reviewed five children?s records and six caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Description:	Accordion expansion gates and pressure mounted gates shall not be used as protective barriers at stair openings. There was a pressure mounted gate used as a protective barrier at the stair openings from the backyard up to the deck.
Plan of Correction:	The provider will replace the gate with one that is not pressure mounted.
