Comments:
An unannounced monitoring inspection was conducted February 5th 2016 between 12:20pm and 1:30pm. There were 63 children in the direct care of 5 staff members. Ratios were maintained throughout the visit. Children were observed playing with electronics, playing gym games, playing with puppets, and building with brick blocks. A sampling of children's records, evacuation drills, the physical space, supervision, and the first aid kit were observed and inspected. The staff were observed working with the children to resolve conflicts by speaking with the children on their level. Standard #: 22VAC40-185-280-G
Description: Based on observation and interview, the center had a hazardous material in a substitute container that did not clearly indicate it's content. Evidence: In room 194 a staff member was using a cleaning solution of bleach and water. The bottle was not labeled with it's contents.

Plan of Correction: Will label the bottle and will indicate the date it was created.
