An unannounced 60 day follow up Inspection to the Provisional License was conducted today November 30, 2015 to check for compliance with Licensing standards. The licensing inspector and licensing Administrator arrived at 9:30 am and concluded the inspection at 12:30 pm. Upon their arrival the caterpillar 1 & 2 classrooms were combined. Children were singing and reviewing numbers on the calendar. Later the class divided and children had center time playing with trucks, house keeping and puzzles. The preschool children were observed having circle time. Children sang songs and discussed the weather. Children also worked on an art project. Children used a Q-tip to paint dots to make the kernels on Indian corn. The Infants were being feed, playing on the floor with toys and napping. Staff interacted with children in a positive manner. Preparation for lunch with hand washing and sanitizing of tables was observed along with lunch time. Children provide their own meals. The facility was inspected to include all rooms. The first aid kit and non-medical emergency supplies were checked and found complete for each floor of the building. The most recent evacuation drill was documented on 11/06/2015. The two required shelter-in-place drills were documented on 10/22/2015 and 11/13/2015. Eight staff record was reviewed along with five children's records. Medication and authorizations were reviewed. There is one staff with current MAT certification on site. Staff have current CPR, First Aid training and Daily Health Observation training. Today the following ratios were observed: Caterpillar 1 and 2: (16 months-2 years) 10 children with 2 staff Cuddle Bugs (Infants) 3 children with 1 staff Butterflies (3 and 4 year olds) 12 children with 2 staff School age children were not in attendance If you have any questions regarding today's inspection, please contact the licensing inspector at (804)662-9758.
Standard #: 22VAC40-185-140-A Description: Based on review of children's records, the facility did not obtain documentation of a physical before the child's attendance or within one month after attendance. Evidence: The record of Child #1(enrolled 10/15/2015), Child #3 (enrolled 10/15/2015), and Child #4 (enrolled 10/15/2015) did not have documentation of a physical in the record. Plan of Correction: Per the Director; I will contact the parent and have them provide documentation of the child's physical for each record.
Standard #: 22VAC40-185-280-B Description: Based on observation the facility did not ensure hazardous substances such as cleaning materials were kept in locked place using a safe locking method that prevents access by children. Evidence: Cleaning materials used to clean and sanitize tables were observed unlocked on a shelf in the toddler room. The shelf was high enough that the cleaning materials were out of reach of children in care. Cleaning materials were observed unlocked on the table in the school age classroom. Children were not in attendance at the time of the observation. Plan of Correction: The Director removed the cleaning materials in the toddler room and placed them in a locked cabinet. The cleaning materials in the school age room were removed and place in a locked cabinet.
Standard #: 22VAC40-185-510-A Description: BAsed on review of medication and medication authorizations, the facility did not obtain written authorization from the parent in order to administer prescription and nonprescription medications. Evidence: The medication for child #7 did not have parent authorization. Plan of Correction: Per the director; I will contact the parent today and request that she complete an authorization form for the medication.
Standard #: 22VAC40-185-510-J Description: Based on observation and interview, the facility did not ensure medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: The Inspector asked the director for any children's medications in order to review as part of the inspection. The director open a desk drawer and provided child #7's medication. The medication was not locked and per interview with the director, she stated that the medication was kept in the unlocked desk in the office. Plan of Correction: The Director stated that she would keep the medication locked in a cabinet.
Standard #: 22VAC40-185-550-F Description: Based on observation, the facility did not have the phone number for the regional poison control center posted in a visible place at each telephone. Evidence: The emergency number list posted at each phone did not have the regional poison control center phone number listed. Plan of Correction: The owner obtained the phone number for the regional poison control center and posted it while the inspector was on site.
