Discussion was held on the topics of current enrollment and current staffing. Discussion was also held on the pending relocation of licensed program while the child care area is under construction/remodeling. Documents are on file and will await the date to change over license to reflect new location. Once work is complete, license will be modified again to reflect original location. Follow up was made with another agency in order to obtain clarification on a report that was received by the center. An error was made by the agency and an explanation was sought. A record search was left blank. A verbal confirmation was obtained.
The following inspection took place between the hours of 1 pm through 3:30 pm. Today, I met with the Staff of the Ashburn Village Sports Pavilion located in Ashburn, Virginia. At the start of this unannounced renewal inspection there were a total of 58 children with 9 staff; good ratios in all 2 rooms. A total of 5 children?s files and 9 staff files were reviewed. The children were engaged in the following: drop-off, check-in, story time, games, large motor activities in the gym, homework time, art, snack, bathroom breaks, hand washing, and other organized activities. Standard #: 22VAC40-185-140-A Description: Based on review, 1 of 5 children's files reviewed indicated that each child did not have a physical examination by or under the direction of a physician within one month after attendance. Evidence - 1. On the date of inspection (02/02/2016), documentation of a physical for Child #3 was not available for review. 2. The start date for Child #3 was documented as 08/31/2015. Plan of Correction: The Parent of Child #3 will be contacted. Once the documentation of a physical is obtained, it shall be placed on file.
