Comments:
Today an unannounced self-reported monitoring visit was conducted with the Center Director regarding concerns about physical abuse of the child. No violations were cited related to those concerns. This visit took place between 9:30-12 there were 9 classrooms observed with a total of 120 children with 22 staff within the supervision guidelines. The children were observed having free play, lunch, story-time and circle time. Positive interactions were observed during this inspection. lunch consisted of tacos, fruit and milk. A complete inspection of the physical plant, children and staff records, fire drill log, menu, and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-270-A
Description: Based on observation of the physical plant, the following area of the center were not being maintained in a clean, safe and operable condition. Evidence: In the 4 year old classroom, the ceiling tile in the bathroom was found hanging from the hinges.

Plan of Correction: Director stated that they are currently in the process of having the tile replaced. During this inspection the violation was corrected and the the tile was replaced.

Standard #: 22VAC40-185-550-D
Description: Based on observation, the center did not have documentation of a monthly fire drill. Evidence: The center did not have documentation of a monthly fire drill for the month of January.

Plan of Correction: Director stated that due to the condition of the weather and sidewalks from the snowstorm they were unable to conduct a drill. Director stated that they will conduct two for the month of February.
