An unannounced inspection for license renewal was conducted today. Ratios for all four groups were found in compliance. The children were finishing lunch, having their diapers changed and transitioning to rest time. Medication procedures were reviewed, and staff with current MAT, first aid and CPR training were on site. Staff and children's records were reviewed today. The outside play area was not checked today because of wet conditions. The exit interview was conducted with the new Head Start Coordinator and the Education Manager. Standard #: 22VAC40-185-60-A
Description: Based on record review, there was no documentation in the record of Child #2, that the required written agreements between the parent and the center were signed by the parent. These agreements are covered in the handbook and there was no documentation that the parent received the handbook.

Plan of Correction: Family Service Worker will obtain written agreement from parent regarding receipt of handbook.

Description: Based on record review, no written physician's authorization to apply a prescription skin ointment was obtained. Findings: At the time of the inspection, the inspector found that there was no written physician's authorization to use a prescription skin ointment for Child #1 on an "as needed" basis.

Plan of Correction: Family Service Worker and Health Coordinator will work collaboratively to obtain physician's authorization to apply prescription skin ointment to child.

Standard #: 22VAC40-185-510-G
Description: Based on record review, no documentation of a MAT certificate was available for a staff person who is administering medication.

Plan of Correction: MAT certification for staff person was retrieved and placed in medication binder.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on record review, no documentation of a central registry search or sworn statement for a new employee was found. Findings: At the time of the inspection, the inspector found that there was no documentation of a sworn statement or CPS central registry search for a new staff person who started on 12/28/2016. The request for the search of the central registry was documented as submitted on 1/15/2016, more than seven days after the employee began working.

Plan of Correction: Sworn Statement and CPS Central Registry search for new staff is in staff personnel file.
