An unannounced monitoring inspection was conducted from 12:12 p.m. - 4:15 p.m. There were no children present at the time of arrival. Later the school bus arrived and there were ten children in care with two teachers. Another school bus arrived and there were 18 children with three teachers present. Three children's records were reviewed. The records for two new staff were reviewed. The children were observed coming into care from school, playing card games, creating with magnet tiles and legos, folding paper making items to play with, playing pretend with the castle, engaged in conversations, preparing for snack and being picked up by their parents. Standard #: 22VAC40-185-240-D-5 Description: Based on record review and interview the facility failed to have a teacher present with current daily health observation training. Evidence: 1. On 02/12/2016 the licensing inspector reviewed the record for two new teachers and there was no documentation of daily health observation training. The facility reviewed the record for another teacher present during the inspection and advised there was no documentation of this staff person having had daily health observation training. 2. On 02/12/2016 staff #1 confirmed that there was no teacher present with daily health observation training. Plan of Correction: We are scheduled to have daily health observation training Tuesday February 23, 2016. Monday we will be in compliance as the teacher who has current daily health observation training will be present.
Standard #: 22VAC40-185-260-B Description: Based on a review of required approval from other agencies the facility failed to have documentation of an annual approval from the health department. Evidence: 1. On 02/12/2016 the licensing inspector had documented having reviewed at the last inspection an approval from the health department dated 08/16/2014. 2. On 02/12/2016 staff #1 provided documentation of an annual health inspection from 2013 and confirmed with the health official via telephone that the last annual inspection had been 08/16/2014. Plan of Correction: A telephone call was made immediately to the health department in order to schedule an inspection.
