During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
- Technical Assistance:

Potable water testing and results completed, sent to VDH. Reminded to review emergency plan annually. Provider is considering becoming a subsidy provider, information shared regarding the process.
- Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 3/27/2025, as a part of the licensure period. The on-site inspection began at 10:55am and ended at 11:55am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 14. The inspector reviewed 3 children?s records and 2 caregiver's records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-750-A Description: When diaper ointment are used, written parent authorization noting any known adverse reactions shall be obtained at least annually.
Provider does not have written authorization noting any known adverse reactions, for use of diaper ointment that has been used on child #2.Plan of Correction: Provider will have parent complete authorization from for diaper cream, with all information, today.
Standard #: 8VAC20-800-50-A Description: The family day home shall keep a written record of children in attendance each day.
There was no record of attendance completed for days provider stated there were children present on 3/20-26 or day of inspection, 3/27.Plan of Correction: Provider will post attendance sheet in a more noticeable place and review daily.
