During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

The following FDH standards were reviewed with the provider:
Standard 140.C - Use of a substitute provider shall be limited to no more than a total of 240 hours per calendar year;
Standard 140.D - A substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works;
Standard 160.C - An assistant shall not be left alone with children in care for more than two hours per day;
Standard 500.F - Trampolines shall not be used during the hours children are in care
The inspector reviewed background check requirements for caregivers. Background checks do not transfer from different Child Day programs operated by a business entity other than the FDH.

An unannounced, on-site monitoring inspection was initiated on 03/25/2025 and completed on 03/25/2025. The on-site inspection began at 12:11 pm and ended at 2:15 pm. The inspector reviewed compliance in the areas listed above. There were 7 children present with 1 caregiver; the point total was 17. A second caregiver arrived at approximately 12:19 pm. The inspector reviewed 7 children's records and 2 caregiver records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 04/8/2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-2 Description: Caregivers shall submit to fingerprinting prior to employment and contact with children.
The results of a fingerprint background check for caregiver 1 was not obtained by the family day home. The fingerprint background check for caregiver 1 was obtained by a different business entity from the licensed family day home.Plan of Correction: Done on 3/26/25.
Standard #: 8VAC20-800-900-B Description: Children shall not be allowed to eat or drink while lying down.
Child 2 was observed napping in a crib with a sippy cup of milk that had an attached lid containing a nipple.Plan of Correction: The cup was removed from the crib.

Assistants' and substitute providers' records shall contain the following:
The file for caregiver 1 lacked a job title, date of employment and name, address and telephone number of a person to be notified in an emergency.Plan of Correction: Corrected

The family day home shall keep a written record of children in attendance each day.
There was no written record of children in attendance when the inspector arrived at approximately 12:11 pm.Plan of Correction: Attendance sheet is on the desk.
Standard #: 8VAC20-800-70-A Description: Repeat Violation
Before the child's first day of attendance, parents shall be provided in writing the following information:
1. The written parent agreement for child 1, 2, 3 and 4 did not indicate the name of the adult assistant or substitute provider ( Caregiver 1) that was caring for children during the inspection.
2. The written parent agreement for child 4 lacked the required information:
a. Policies for termination of care;
b. General daily schedule that is appropriate for the age of the enrolling child;
c. Schedule of fees and payment plans.Plan of Correction: This was not a repeat violation on the same child. All children records have been corrected.
Standard #: 8VAC20-800-240-A Description: Areas and furnishings of the family day home, outside, shall be maintained in a safe condition.
Standing water, broken equipment and trip hazards were observed on the outdoor play area. All of these were accessible and within reach of children.Plan of Correction: Yard was cleaned up. The yard had standing water and the top of the playhouse has blown off due to a storm.
Standard #: 8VAC20-800-530-A Description: Cribs, cots, rest mats when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover or a one-piece covering that is open on three edges.
Children were observed napping on rest mats and in cribs without any top cover and bottom cover.Plan of Correction: Top and bottom covers were provided while inspector was here.
Standard #: 8VAC20-800-570-A Description: The provider shall ensure that a caregiver does not exceed 16 points by using the following point system to determine if an additional caregiver is needed:
*Children from 16 months through 23 months of age count as three points each;
*Children from two years through four years of age count as two points each.
Caregiver 1 was observed alone with 7 children whose ages counted for a total of 17 points.Plan of Correction: A new employee started on 4/2/25. There will be two assistants at all times.
