Technical Assistance:
**Staff records were locked in the HR office in a filing cabinet inaccessible to the staff on duty. Will review staff records during the next inspection.

Comments:
An unannounced monitoring inspection was conducted today at 4:45pm. There were 34 (Preschoolers and School-agers) directly supervised by 6 staff. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in individual activities. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-150-B
Description: Based on review, the facility failed to ensure that the immunization records are signed by a physician, their designee or an official of a local health department. Evidence: Reviewed 5 children's records and found that CH1 did not have documentation of a signed immunization record.

Plan of Correction: The parent is bringing in anew, updated and signed immunization form.

Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 5 children's records and found... 1) CH3 did not have documentation of a second emergency contact. 2) CH1 did not have documentation of the parent's annual updated of emergency contact information. CH1's last update was on 8/21/14. 3) CH2 did not have documentation of an emergency medical care authorization form.

Plan of Correction: The parents were immediately contacted and each records was updated accordingly.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Observed in the Preschool Room unlocked cabinets with shampoo, dish soap, hand sanitizer and cocoa butter, marked keep away from children. 2) Observed in the K-3 Room Dawn soap accessible to children in a lower unlocked cabinet. 3) Observed in the 4-8th grade rooms, 8 lotions, 1 hand sanitizer and 1 container of disinfectant wipes accessible to children on various tables, desks and counter tops.

Plan of Correction: 1) Items were put away in a locked cabinet out of reach of children. 2 + 3) Items were removed from classroom.
