During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. - Technical Assistance:

* Discussion that the Provider will forward the confirmation email form the Virginia Department of Health - Office of Drinking Water that the lead testing results that were sent had been received. Currently, this FDH location is not listed on the water testing webpage.
* Effective 12/09/2024, the Office of Background Investigations implemented changes to their provider portal. Please note that all providers will need to create an account to request Central Registry background clearances. Please contact OBI directly if you have questions or concerns. Here is a caption from the memo that was sent on 12/04/2024: Both current and new users will need to register a new account to access the new OBI Provider Portal after December 9, 2024, to ensure all features of the enhancements are applied to user accounts. Registration will create an account with up to 5 authorized users and will allow the primary account user to update account access and facility contact information as needed.

An unannounced, on-site monitoring inspection was initiated and completed on 04/01/2025. The on-site inspection began at 10 am and ended at 12 pm. The inspector reviewed compliance in the areas listed above. There were (11) children present and (4) caregivers; the point total was (38). The inspector reviewed (11) children?s records, (4) caregivers, and (3) household members records on-site on 04/01/2025.
This inspection included: document review (i.e. files, Injury logs (no reports filed), policies and procedures, emergency drill logs, authorization forms); tour of the facility; interviews; and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-780-B Description: Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review.
The record for Child #8 did not have a current parent?s signature for an annual review of the emergency contact information. The last documented review was over 13 months ago.Plan of Correction: The emergency information will be reviewed and a signature and date will be obtained as verification.
Standard #: 8VAC20-800-60-B Description: Children's records are required to contain the policy number of the child's medical insurance,.
The record for Child #11 was missing the policy number of Child #11's medical insurance. The child was present and had started care of the date of inspection.Plan of Correction: Corrected on the date of inspection. The Provider was able to obtain this information after the inspection had concluded.
Standard #: 8VAC20-800-70-B Description: Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information regarding family day home operations and administration.
There was no parent acknowledgement available for Child #11 in regard to the FDH's Decision to Administer. Child #11 started care today and was present during the inspection.Plan of Correction: A Medication Administration - Decision to Administer form will be reviewed and signed by the parent of Child #11.
