An unannounced, on-site renewal inspection was initiated on 4/9/2025 and completed on 4/9/2025, as a part of the licensure period. The on-site inspection began at 9:18am and ended at 10:40am. There was one child present and one caregiver; the point total was two. The inspector reviewed two children?s records and three caregiver/household members records on-site. This inspection included document review and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

When meals or snacks are provided by the family day home, the menu for the current one-week period must be dated and given to parents, posted, or placed in an area accessible to parents.
The menu posted for parents was outdated by fifteen weeks.Plan of Correction: Will post a weekly updated menu for parents.
Standard #: 8VAC20-800-100-A Description: REPEAT VIOLATION
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
There was no physical exam record available for Child #2, who had been in care for two years and two months.Plan of Correction: Will obtain physical from the parents.

Pet A and Pet B's rabies vaccinations were outdated by four months.Plan of Correction: Will schedule an appointment to get the vaccinations for the two pets.
