22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring visit and facility tour was conducted from 3:10 pm-4:15 pm. At the time of inspectors' arrival there were 47 preschool and school aged children in care with 7 staff members and 1 administrator. Children were observed participating in snack, handwashing, outdoor play, transitioning between activities, and nap. A sample of 5 children's records were reviewed. Medications and injury reports were also reviewed. A sample of 3 staff records were reviewed on 2/5/16 and 2/10/16 at the administration offices. Areas of noncompliance are noted on the violation report and were discussed during the exit meeting on the day of the inspection with the Director. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean and safe condition. Evidence: The little tikes playhouse on the playground that had large amounts of spider webs, leaves, and debris in it. The director noted that they plan to move this out of the playground area due to the above noted reasons.

Plan of Correction: A request was placed to the facility supervisor for the dismantling and removal of the Little Tike playhouse from the Early childhood playground on 2/3/16. The request was sent out and completion of the dismantling and removal should be By 2/19/2016. In the meantime the area is roped off and children have not and will continue not to play in the house.

Standard #: 22VAC40-185-550-M
Description: Based on record review it was determined that the center did not ensure that all written records of children's serious and minor injuries contained all of the required components. Evidence: 1. The injury report dated 1/22/16 for child #6 only contained one of the two required signatures. 2. The injury report dated 1/29/16 for child #7 only contained one of the two required signatures. 3. The injury report dated 1/13/16 for child #8 only contained one of the two required signatures. 4.The injury report dated 1/28/16 for child #6 did not contain any future action to prevent recurrence.

Plan of Correction: Staff were reminded and retrained on 02/04/2016 on how to completely fill out major minor injury report form. Staff was also reminded on the importance of accurate documentation.
