An unannounced, on-site monitoring inspection was initiated on 4/14/2025 and completed on 4/14/2025. The on-site inspection began at 1:55PM and ended at 3:15PM. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 12. The inspector reviewed 3 children?s records and 2 caregiver/household member records.
This inspection included document review, a tour of the facility, interviews, observations.

Standard #: 8VAC20-800-120-B Description: Assistants' and substitute providers' records shall contain the following: Documentation of the education and training as required by 8VAC20-800-230.
Verification of education and programmatic experience was not available for review for Caregiver B.Plan of Correction: We will get the information right away and place in the file.
Standard #: 8VAC20-800-140-B-3 Description: Providers shall have a current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.
The CPR certification for Caregiver A expired approximately 2 weeks ago.Plan of Correction: A class has been scheduled for this upcoming Saturday.
