A staff record review was completed at the central location. Two staff records and four board members/agent records were reviewed. A violation was found in the area of training and appears on the violation notice.
Based on record review and interview the center failed to ensure that staff obtain the required number of annual training hours.
1 - The record for staff 1 contains documentation for 5 and 1/4 hours of annual training where 16 hours are required.
2 - Staff 2 confirmed that there were 5 and 1/4 hours of training documented.
Plan of Correction:
Agency management will ensure that the required number of training hours are obtained and documented in each staff record.
