An unannounced, on-site monitoring inspection was conducted on 4/21/25 from 10:40 am to 11:40 am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 17. The inspector reviewed 2 children's records, 2 caregiver's and 2 household members records on-site. This inspection included document review, tour of the family day home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-180-A Description: The provider shall obtain for each adult household member a current report of Tuberculosis (TB) Screening form, every two years.
The TB screening for the Household Member 1 was last completed on 11/24/22. A subsequent TB screening has not been obtained to this date.Plan of Correction: The screening has been scheduled for 4/24/25.
