Comments:

An unannounced, on-site renewal inspection was initiated and completed on April 21, 2025, as a part of the licensure period. The on-site inspection began at 10:15am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 11 children present and two caregivers; the point total was 28. The inspector reviewed seven children?s records and three caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-800-830-C
Description:	Documentation shall be maintained of emergency evacuation drills. Documentation for emergency evacuation drills were not maintained for the months of January, February, and March.
Plan of Correction:	Moving forward, the provider will ensure to document all monthly emergency evacuation drills.
Standard #:	8VAC20-800-90-C
Description:	The family day home shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1 did not have documentation of additional immunizations. The last documentation of additional immunizations was dated 02/17/2023, when Child #1 was 12 months old. Child #1 has been in care for 35 months.
Plan of Correction:	The provider will contact the parent of Child #1 and obtain a copy of the current immunizations. Moving forward, the provider will ensure to obtain current immunizations for all children.
Standard #:	8VAC20-800-210-A
Description:	Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Caregiver #2 obtained 5.5 hours of 16 hours of annual training in areas relevant to their job responsibilities.
Plan of Correction:	Caregiver #2 will complete 11.5 hours of annual training in areas relevant to their job responsibilities.
