11 children present with 3 staff in two groupings. The children were observed having circle time and singing lots of songs, working on colors and cutting skills in small groups, studying dinosaurs and matching bones to animals. The children were also observed playing on the playground. Lots of interaction between staff and children observed during the visit. This visit was started at 9:45 and ended at 11:15. Standard #: 22VAC40-185-140-A Description: The center failed to get a physical examination report for one child out of 6 children's files reviewed. Evidence: Child A is missing a physical. Plan of Correction: Will ask the parent for the physical.
Standard #: 22VAC40-185-60-A Description: The center failed to get some of the required information for children's files. Evidence: Based on the review of 6 children's records the following was found missing; Child A is missing the mother work information. Children B,D,E and F are all missing some part f the emergency contact information, child B has the parents listed, child E is missing the second emergency contact and the rest are missing the addresses for the emergency contacts. Plan of Correction: I will ask the parents for the information missing in the files.
