Consultation was given on VA DSS model forms located on the VA DSS web site.
An unannounced monitoring visit was conducted from 11:30-1:15. Children were observed diapering, transition to the playground, walks and preparation for lunch. All rooms are well supplied with materials and equipment. Staff record of newest hire checked as well as emergency preparation, physical space, diapering supplies and ratios and supervision. Medication is on-site and current. Lunch boxes were dated and labeled. Each child brings a lunch from home and staff were observed hand washing, eating with children and reading stories during transition. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-260-A Description: Based on observation, the current fire permit is expired. Evidence: The fire permit expired 1/31/2016. Plan of Correction: Call for re-inspection.
Standard #: 22VAC40-185-260-C Description: Based on current asbestos report (dated 2003), a follow up visit every three years is required and has not been conducted. Evidence: Based on the current asbestos documentation, a follow up visit must be completed every three years to review areas where asbestos is notes. No documentation is available to determine if any follow up visits have been completed or is required. Plan of Correction: Contact trustees of the church to update information.
Standard #: 22VAC40-185-550-D Description: Based on evacuation log, a monthly fire drill was not completed. Evidence: An evacuation drill was not conducted the month of January. The Center was not open for 7 school days due to snow closures as reported by the Center Director. Plan of Correction: Conduct fire drill.
