An unannounced monitoring inspection was conducted on February 19, 2016 from approximately 7:50am to 9:00am. At the onset of inspection, two staff were present with 15 children. Throughout the course of the inspection there were 26 children with three staff members. Children were observed playing with tabletop activities and during open play in the gym. Three children's records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, hand washing, nutrition, injury report documentation, medication procedures, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Violations were reviewed with and verified by Staff #1.
Standard #: 22VAC40-185-240-D-4 Description: Based on observation and inspection of the facility, the licensee did not ensure that any child for whom emergency medications (such as but not limited to albuterol, glucagon, and epipen) have been prescribed shall always be in the care of a staff member or independent contractor who meets the requirements in subdivision 1 of this subsection. E vidence: During the inspection conducted on February 19, 2016, an emergency medication was prescribed for Child #1 who was present on the date of the inspection. Staff #1 and Staff #2 were present but neither staff person had documentation of MAT training. Plan of Correction: A MAT trained staff was sent over to be on site as needed per licensing. MAT trained staff will continuously be on site as needed per licensing.
Standard #: 22VAC40-185-510-C Description: Based on observation and inspection of the facility, the licensee did not ensure that the center's procedures for administering medication shall have methods to prevent use of outdated medication. Evidence: During the inspection conducted on February 19, 2016, there was an expired medication for Child #2 in the medication box, with instructions that the medication was to be discarded after 12/11/15. Plan of Correction: The medication was removed immediately and the parent was contacted to pick up expired medication.
