During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated on April 29 2025 and completed on April 29 2025. The on-site inspection began at 1:30pm and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There was 1 childr present and 1 caregiver; the point total was 4. The inspector reviewed 1 child records and 3 caregiver/household members records.
This inspection included document review, a tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on May 22 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-770-40-D-5 Description: A person living in the home, 14 years of age and older, must have a search of the central registry and make the information available for regulatory purposes within 30 days of a person in the home becoming 14 years old.
Household member # 2 did not have a central registry search completed within 30 days of the person becoming 14 years of age. The provider was out of compliance for 3 weeks.Plan of Correction: Not available online. Contact Inspector for more information.
