During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was completed on 5/5/2025, as a part of the licensure period. The on-site inspection began at 9:55 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above. There were (3) children present and (1) caregivers; the point total was (8). The inspector reviewed (4) children?s records and (3) caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-800-800-B Description: REPEAT VIOLATION
The provider shall review the emergency plan at least annually and update the plan as
needed. The provider shall document in writing each review and update to the emergency
plan.
The provider has not documented in writing the annual review of the emergency plan. The
annual review is overdue by 12 months.Plan of Correction: Provider will review the emergency plan, sign and date it.
Standard #: 8VAC20-800-830-C Description: REPEAT VIOLATION
Documentation shall be maintained of emergency evacuation and shelter-in-place
drills.
There was no documentation of emergency evacuation drills for the year 2025.Plan of Correction: Provider will conduct monthly emergency fire drills and document properly.
Standard #: 8VAC20-800-100-A Description: The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. T
here were no physical exam records available for Child #3, and #4 , who had been in care for 3 months.Plan of Correction: The parents will obtain a physical examination and it will be filed in the child's record.
Standard #: 8VAC20-800-60-B Description: REPEAT VIOLATION
Children's records are required to contain name, address and phone number of 2
persons to call in an emergency, proof of child's identity, and documentation of the review of the child's emergency contact information.
The record for child #1 was missing a second name address and phone number to call in an emergency and the annual review of child's emergency contact information. The child was
present during this inspection and has been in care for 22 months.
The record for Child #2 was missing name, address and phone number of 1person to call
in an emergency, and proof of child's identity. The child has been in care for 10 months.
The record for child #3 was missing the second emergency contact address and the proof of birth. The child was present during this inspection and has been in care for 3 months.
The record for child #4 was missing the second emergency contact address and the proof of birth. The child was present during this inspection and has been in care for 3 months.Plan of Correction: Provider will have the parents complete all the required missing information.
Standard #: 8VAC20-800-70-A Description: REPEAT VIOLATION
Before the child's first day of attendance, parents shall be provided required information
in writing.
The parents for child #2 were not provided in writing the following required information:
policies for administering medications and whether or not there is liability insurance. The
child has been in attendance for 10 months.Plan of Correction: Provider will provide the missing information to the parents for a signature and file in the child's record.
