Comments:
A scheduled initial inspection was conducted today by the licensing inspector. The center owner/director was present during the inspection. There were no children in care today. Some but not all standards could be determined at this time. The facility was set up with program materials, required posted items, sign in sheets, first aid supplies and the phone is in service. There is a warming kitchen on site and toilets/sinks to support the requested capacity. There is a sink in the classrooms where diapering will be done.
Supervision and the need for a program leader in each group was discussed with the applicant. There is shade along the perimeter of the playground and a large tree from an adjoining yard may also provide shade. The possibility of additional shade being needed during the summer was discussed. There is a qualified program director to be on site at least 50% of operating hours.
Sample forms provided by the licensing division will be used by the facility. Once children have been enrolled, their records will be reviewed at a subsequent inspection. The records for the owner/program director and a program leader/director were reviewed on site. Transportation will not be provided.

Based on square footage, 50 children may be on the playground at one time. Children under 2 yrs of age will use the playground at a scheduled time, using a designated area, when the other children are not on the playground. There is no fixed equipment on the playground at this time.

Infant cribs have a manufacture date of 7/2014.

Description: Based on inspection, the applicant did not ensure the facility is maintained in a clean and safe condition.
Evidence: Playground - The post holes at the bottom of the fire escape have not been filled in. The fencing on the playground has numerous areas that are rough, and splintering, loosely attached to the support boards. The door of the upstairs preschool bathroom closes very quickly such that a child might not be able to hold the door open when entering/exiting the bathroom.
Some type of alarm is needed at the upstairs door leading to the fire escape ensuring that a child doesn't go out to the fire escape unattended.

Plan of Correction: The fencing and sanding of boards, securing loose boards will be done by 3/4. The post holes will be filled in today. I will install an alarm on the door by 3/4 /16. Everything will be completed by 3/7/16.
