22VAC40-185 STAFFING AND SUPERVISION.
An investigation was conducted today in response to a self-reported incident received by our office regarding the care and protection of an infant. Based upon documentation reviewed and multiple interviews, the preponderance of evidence does support a violation. The exit interview was conducted with the Director. Standard #: 22VAC40-185-340-A Description: Based upon multiple interviews, a staff did not ensure the care and protection of an infant when she fed her another infant's breast milk bottle. Evidence: On February 17, 2016 at approximately 4:45pm, Staff A fed Child A (8 months) a mixture of breast milk and formula bottle that belonged to Child B (6 months). Staff A confirmed she grabbed the bottle that belonged to Child B and checked the name written on the bottle that identified the owner; however, she only read the last three letters. As a result, Child A drank more than half of a bottle that belonged to Child B. Staff A did not realize she had fed Child A the wrong bottle until another staff identified Child B's bottle. Plan of Correction: Re=trained staff on proper feeding and accurate bottle identification.
