Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Discussed MAT and authorizations, acknowledgement forms, weekly menu and first aid manual. Inspector assisted provider with submitting lead testing results to VDH.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on May 5, 2025. The on-site inspection began at 10:25am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were ten children present and three caregivers; the point total was 32. The inspector reviewed five children?s records and four caregiver/household members records on-site. This inspection included document review, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Description:	Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, shall be stored in areas inaccessible to children. There was a bottle of hand sanitizer on the sink in the children's bathroom. There was a second bottle of hand sanitizer in the children's art room on a desk.

Standard #:	8VAC20-800-290
Description:	Sharp kitchen utensils and other sharp objects shall be inaccessible to children unless being used by the caregiver or with children under close supervision. A pair of adult scissors was found in the children's bathroom in an unlocked drawer at the bottom of the bathroom sink. A second pair of scissors was found in the children's art room on the children's art table.
