Technical assistance was given regarding the clear documentation of emergency contact addresses.
An unannounced monitoring inspection was conducted on March 1, 2016. There were 2 staff were present with 16 children. Children were observed playing on the outside basketball court and the playground. Children were also observed preparing for and eating snack. The sample size of records reviewed included 3 children's records and the health related information and emergency contact information for 2 staff members. Four medications were reviewed as a part of the inspection. The program was relocated and held in the library. The inspection began at approximately 4:10 PM and ended at 6:05 PM. The results of the inspection were reviewed with the center director during the exit interview.
Standard #: 22VAC40-185-140-A Description: Based on a record review and staff interviews, the licensee did not ensure there was a copy of a physical examination conducted under the direction of the physician. Evidence: During the inspection conducted on March 1, 2016, the record for child #3 did not include a copy of a physical examination. The form the center staff believed was the physical did not include documentation that a physical was conducted under the direction of a physician as it did not include documented results from the physician or the physician's signature or signature of the physicians designee or the physicians stamp on the physical form. Plan of Correction: The center director will contact the parents for an updated physical form.
Standard #: 22VAC40-185-40-D Description: Based on observation and staff interviews, the licensee did not ensure the license was posted in a place conspicuous to the public. Evidence : During the inspection on March 1, 2016, the most recent license was not posted in a conspicuous to the public. The program had been relocated to the library for the day and the center supervisor said "the licensing board was in the cafeteria and they were not allowed to go in there because of voting". Plan of Correction: The center supervisor will take the license when the program is relocated and post it .
Standard #: 22VAC40-185-550-C Description: Based on observation and staff interviews, the licensee did not ensure the emergency evacuation and shelter-in-place procedures/maps were posted in a location conspicuous to staff and children. Evidence : During the inspection on March 1, 2016, there was not an evacuation map or procedure posted in library where the after school program had been relocated. Plan of Correction: The center director will copy document to have ready for future relocation.
Standard #: 22VAC40-185-560-K Description: Based on observation and staff interviews, the licensee did not ensure the tables were sanitized before and after each use for feeding. Evidence : During the inspection on March 1, 2016, the children were observed eating snack and the tables were not sanitized after eating. The center staff was observed wiping the tables with disinfectant wipes after the children were finished eating snack but the tables were not sanitized as required. The definition of "Sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of the item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution. Plan of Correction: The center director will ensure to spray the tables.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation and staff interviews, the licensee did not ensure the findings of the most recent inspection of the facility were posted in a conspicuous place for the public to view. Evidence : During the inspection on March 1, 2016, the most recent inspection findings were not posted. The program had been relocated to the library for the day and the center supervisor said "the the licensing board was in the cafeteria and they were not allowed to go in there because of voting".
