A complaint was received by The Department of Social Services Central Regional Licensing office on February 19, 2016 concerning documentation of records and physical plant. The licensing Inspector conducted an unannounced inspection in response to the complaint on February 24, 2016. The Inspector was on site from 11:00 am until 12:00 pm. The facilities Director/Owner and staff were interviewed, documentation of children's immunizations and physicals were reviewed (see violation notice) and the physical plant of the facility was inspected. Based on the information gathered during the investigation, the preponderance of evidence does not supports the allegation, therefore the complaint is not valid. Description: Based on review of children's records, the facility did not obtain documentation of a physical before the child's attendance or within one month after attendance. Evidence: 1. The record of child #1 with an enrollment date of 10/14/2015 did not have documentation of a physical exam on record. 2. The record of child #2 with an enrollment date of 10/14/2015 did not have documentation of a physical exam on record.

Plan of Correction: Per the Director; I will contact the families and request that they provide documentation of a physical exam for the children's record.
