54.1 Provider must be MAT certified to administer prescription medication.

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
1. Household products labeled "keep out of reach of children" shall be stored in this manner. These products should not be accessible to children in care.
2. The inspector provided the VDH email address for the final step of water lead testing.
3. Recommend caregivers attend Family Day Home Phase 2 Training.

Comments:
An unannounced, on-site renewal inspection was conducted on 5/16/25. The on-site inspection began at 11:09am and ended at 12:26pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 3 caregivers; the point total was 5. The inspector reviewed 5 children?s records and 4 caregiver/household member(s) records while on-site. The inspection included document review, tour of the home and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/23/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-B-2
Description: Each caregiver must complete fingerprinting/criminal record check (CRC) prior to the first day of employment.

Substitute caregiver #2 began working on 4/17/25 and does not have a completed CRC. It was reported that the licensee has not requested a CRC.

Plan of Correction: CRC was filed yesterday. But going forward she won't be helping anymore

Standard #: 22.1-289.035-B-3
Description: Each caregiver must complete a central registry search (CRS) for child abuse and neglect search prior to the first day of employment.

Substitute caregiver #2 began working on 4/17/25 and does not have a completed CRS. It was reported that the licensee has not requested a CRS.

Plan of Correction: CRS was filed yesterday. But going forward she won't be helping anymore

Standard #: 22.1-289.035-B-4
Description: Each caregiver that has lived out-of-state in the previous 5 years must complete a criminal record check (CRC) and central registry search (CRS) specific to that State, and a national sex offender search prior to the first day of employment.

Substitute caregiver #2 began working on 4/17/25 and does not have a completed CRC from the State listed on the sworn disclosure statement and a sex offender search. It was reported that the licensee has not requested these background checks.

Standard #: 8VAC20-800-1000
Description: The provider shall ensure that vehicles used for the transportation of children shall meet the safety standards set by the Virginia Dept. of Motor Vehicles.

The vehicle used to transport children is not tagged in Virginia and has not had a Virginia safety inspection.

Plan of Correction: Going forward will be using a different car for transport, registered under the state of VA

Description: Assistants' and substitute providers' records shall contain the following

The record for substitute caregiver #2 did not include date of employment; name, address and phone number of an emergency contact; a minimum of 2 written reference checks and documentation of the education and training required for a substitute caregiver.

Description: The family day home shall keep a written record of children in attendance each day.

There was no record of attendance for Child #7 who was in attendance today.

Standard #: 8VAC20-800-60-A
Description: The provider shall maintain an up-to-date record at the family day home for each enrolled child.

There was no record of enrollment for Child #7 who was in care today.

Standard #: 8VAC20-800-60-B
Description: A child's record shall contain the name and policy number of the child's medical insurance.

There was no documentation of the policy number for Child #6. The space on the form was left blank.

Plan of Correction: Correction has been made and added to child #6 folder

Standard #: 8VAC20-800-90-C
Description: The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization requirements every 6 months until the age of 2 years old.

The immunizations on-file for Child #6 were dated July 2024 when the child was 3 months old.

Plan of Correction: Correction has been made and added to child #6's folder

Standard #: 8VAC20-800-140-D
Description: A substitute provider shall record and sign the time of arrivals and departures on each day that the substitute provider works.

The timesheets for substitute caregiver #1 and #2 have not been maintained on a daily basis. Both reported to be working daily since 4/17/25 and were working today however, the last timesheet entry for working for both was 5/8/25.

Plan of Correction: started using the correct timesheet immediately

Standard #: 8VAC20-800-240-A
Description: Areas and furnishings of the family day home, shall be maintained in a clean and safe condition.

A cell phone and purse were observed lying in a crib reported to be the sleep space for Child #6.

Plan of Correction: Corrected immediately on 5/16/2025
