- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection March 16, 2016 from approximately 10:20am to 1:30pm. The following ratios were observed with a total of 46 children present during the inspection: Infants - 10 children with 3 staff Ones - 4 children with 1 staff Twos - 12 children with 2 staff Threes (Preschool) ? 3 children with 1 staff Threes II (Preschool) ? 7 children with 1 staff Fours & Fives (Pre-K) - 10 children with 1 staff School Age children were not present during the inspection. During the inspection, the Infants were observed having their diapers changes, playing on the floor with toys, eating and sleeping in their cribs. The Ones were observed having a book read to them and coloring. The Twos classroom was observed reviewing flash cards, making animal sounds and creating St. Patrick?s Day hats. The Threes classrooms were observed playing and eating lunch. The Fours and Fives were observed washing hands, eating lunch, creating boy /girl patterns, playing with dinosaurs and building with blocks. Staff was observed interacting with the children and providing assistance when needed. Lunch consisted of corn dog nuggets, macaroni and cheese, corn, pineapples and milk. Later children began to transition into naptime. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, daily sheets, parent agreements, children's assessments and staff training information. The owner stated medication is not administered at this time. During the inspection, five children and five staff records were reviewed. Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, one record was missing the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #3 (enrollment date 03/07/16) did not contain the physical address for one of the two emergency contacts listed. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-70-A Description: Based on review of five staff records, one record was missing the required information. Evidence: During the inspection, the licensing inspector observed the record for Staff #4 (hire date 01/04/16) did not contain documentation of orientation prior to the staff member assuming job responsibilities. Plan of Correction: Per owner - the orientation was completed on the staff member's hire date. The documentation of the orientation could not be located during the inspection. We had the staff member sign another orientation form while the inspector was on site.
Standard #: 22VAC40-185-440-J Description: Based on observation, the center did not ensure the required spacing for occupied cribs was maintained. Evidence: During the inspection, the licensing inspector observed four cribs that were occupied with sleeping infants. The four cribs were approximately 12 inches apart or less on both sides of the cribs. Child Day Centers standards require at least thirty inches of space between service sides of occupied cribs for staff to gain access. Plan of Correction: Per owner - cribs were moved during the inspection.
Standard #: 22VAC40-185-570-A Description: Based on observation, the center did not ensure children under 12 months of age were securely fastened in a high chair. Evidence: During the inspection, the licensing inspector observed two infants under 12 months of age sitting in high chairs. The infants did not have a protective belt securely fastened. Plan of Correction: Per owner - the padding and belts for the high chairs were just replaced and the belt had not been fastened. Staff were instructed to loop the belts through the high chairs during the inspection.
