22VAC40-191 Background Checks (22VAC40-191)
Today an unannounced renewal visit was conducted with the Center Director. This visit took place in the mid morning/afterrnoon between 11:15-2:00pm. There were 2 classrooms observed with a total of 24 students with 5 staff within the supervision guidelines. The children were observed having morning snack, circle time, outdoor free play, lunch and in transition for dismissal. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, insurance, and injury report and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-270-A Description: Based on observation of the physical plant, the following area of the center were found not being maintained in a clean, safe and operable condition. Evidence: Upon arrival, rusting & a hole was found along the stairs/railing leading to the back door entrance . Plan of Correction: Center Director stated that she has placed a work order to the landlords and they have placed an order to have the whole stairwell replaced. Center Director also stated that this entrance/exit is currently not being used by the children until it is fixed.
Standard #: 22VAC40-185-550-M Description: Based on a review of the center written record of injuries, the form being used were missing the following information: any future action to prevent recurrence, Documentation on how parent was notified. Evidence: Child #1injury report form did not include future action to prevent reoccurrence and documentation on how parent were notified. Plan of Correction: Center Director printed a copy of the state model form from the website and added the missing information to the form.
Standard #: 22VAC40-191-40-D-1-C Description: Based on observation of the staff records, one record did not include an updated central registry check within three years from the date of the last screening. Evidence: Staff #1's record central registry check was beyond the expiration date (8/12/15). Plan of Correction: Director stated that the CPS check was returned because she sent a personal check. Director stated that the form was resubmitted on 2/26 and is waiting for it to be returned.
