22VAC40-185 STAFFING AND SUPERVISION.

Standard #: 22VAC40-185-340-A Description: Based on interviews, staff did not ensure care, protection, and guidance while supervising a group of children. Evidence: On 03/07/2016, staff #1 & #2 were supervising a group of 19 children. Staff #1 gathered the group of children and assisted them with putting on coats in transition for outdoor time. Child #1 went to the rest room, while Staff #1 assisted the other children to line up and prepare for outdoor play. Staff #1 & #2 walked the children from the classroom to the outdoor playground. As they entered the playground and began counting the children they noticed Staff #3 walking to the playground with Child #1. Staff #1 & #2 failed to provide care, protection and guidance when a 4 year old child from the group of children was not accounted for and was found by Staff #3 alone running to the front door & then back to the classroom trying to find his class. Plan of Correction: Director stated that staff will be retrained on transitions, head counts, face to name and checking the hot spots in the classroom before exiting the class.
Standard #: 22VAC40-185-340-F Description: Based on interviews, a child under 10 years of age was found not within sight and sound supervision. Evidence: On 03/07/2016, a (Child #1) 4 year old was found alone running up the hall towards the front door trying to locate his class. Staff #3 immediately assisted him in locating his class who were on the playground. Plan of Correction: Director stated that staff will be retrained on transitions, head counts, face to name and checking the hot spots in the classroom before exiting the class. Director will also had staff shadow other staff to observed how they transition and ensure accurate counts of children.
