An exit interview was conducted with the director to discuss the results of today's inspection.
An unannounced monitoring inspection was conducted today, March 18, 2016 to determine the center's compliance with licensing standards. Upon arrival, one two year old child was observed being supervised by two staff. The child was observed playing with toys, singing songs, and interacting with staff. All areas used by children were inspected to include the main hallways, children's restrooms, classroom, and office.
The program had the required postings displayed on the wall upon entering the office. The center's first aid kit and emergency supplies were inspected with the required supplies found. One child's and one staff record were inspected with the majority of the required information present. The following was also inspected on this day: annual fire & health inspections, fire/shelter-in-place drills, emergency preparedness plan, injury prevention plan, parent agreements, and other pertinent information. See attached notice for violations found during this inspection.

Standard #: 22VAC40-185-550-A Description: Based on the inspection of the emergency preparedness plan, the required components were not completed. Evidence: During the inspection of the emergency preparedness plan, it was discovered various sections were not completed such as emergency telephone numbers, designated staff, and staff duties. Plan of Correction: Per director: I will start completing the incomplete section of the emergency preparedness plan.
Standard #: 22VAC40-191-40-D-1-C Description: Based on the inspection of staff records, the required sworn statement or affirmation was expired.
During the inspection of one staff records, it was determined that the sworn statement dated 9/4/12 expired on 9/3/15. An updated sworn statement or affirmation should have been completed prior to expiration.
Plan of Correction: Per director: I will download the form off the social services website and complete it.
