Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 5/27/25. The on-site inspection began at 3:15pm and ended at 4:15pm. The inspector reviewed compliance in the areas listed above. There were six children present and two caregivers; the point total was 19. The inspector reviewed three children?s records, two caregiver records, and two household member records on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/6/25. A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-800-60-B
Description:	Child records shall contain all of the required information. The record for Child A did not contain documentation of the name, address, and phone number of a second emergency contact who can be reached in case of an emergency if the parent cannot be reached.
Plan of Correction:	Child A record will be completed.
Standard #:	8VAC20-800-180-A
Description:	The provider is required to obtain for each caregiver and adult household member a current tuberculosis (TB) test or screening every two years from the date of the first screening. 1. The TB screening on record for Caregiver #1 was past due by seven weeks. 2. The TB screenings on record for Household Members #1 and #2 were past due by more than six weeks.
Plan of Correction:	(TB) test for the caregiver and the other household members will be obtained.
Standard #:	8VAC20-800-540-A
Description:	Infant carrier seats, swings, strollers, feeding or activity tables, and high chairs are required to be used according to the manufacturer?s instructions and when occupied by a child, a safety strap is required to be used and securely fastened. The table and highchairs were approximately 27? tall. On 5/27/25 an 11-month-old and 13-month-old were observed sitting in a feeding/activity table with the straps removed so the children were not securely fastened. A 1-year-old and a 23-month-old were observed sitting in highchairs without the safety straps securely fastened.
Plan of Correction:	Safety strap will be used and always securely fastened to any highchair that 27 inch tall.
