An unannounced inspection was initiated and completed on 05/28/2025 in response to a complaint received by the licensing office on 05/21/2025 relating to allegations of 8 VAC 20-800.30.H. This inspection included: document review, tour of the facility; interviews; and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violation with an applicable standard or law. No violation was issued.
