Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on March 22, 2016 from 11:00 a.m. through 2:00 p.m. On this date, there were thirty-five (35) children in the care of nine (9) staff members in five classrooms. There are a total of sixty-eight (68) children enrolled in this program. Five (5) children's records and eleven (11) staff records were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Areas observed were hand washing/bathroom procedures, transition, story time, lunch and nap. Lunch was observed to meet USDA guidelines and consisted of spaghetti with meat sauce, spinach, peaches, and milk. All interactions between staff and the children were appropriate and the children seemed comfortable with the daily routine. There are a wide variety of age/stage materials for the children in care. Evidence: Based on record review of five children's records, one record for Child D did not have documentation of the physician's name and phone number. Child D did not have documentation of a second emergency contact. Evidence: Based on record review of five children's records, one record for Child D did not have documentation of a second emergency contact. Plan of Correction: The director will contact the parent to verify and document a second emergency contact and the name of the child's physician and phone number.
Standard #: 22VAC40-191-40-D-1-C Description: Staff A did not have documentation of obtaining an updated criminal record check. Evidence: Based on record review and conversation with the provider, it was determined Staff A did not have documentation of obtaining an updated criminal record check. Last documented updated was 06/11/2012. Plan of Correction: The director will complete an updated criminal record check on Staff A. In the future, all repeat checks will be conducted every three years.
