Today we discussed fire inspections and site coordinator responsibilities.

An unannounced monitoring inspection was conducted today between 9:40am and 12:40pm. There were 45 children in care with six teaching staff staff and two administrators. Children were observed in teacher and self-directed activities including but not limited to center play choices, circle time, breakfast, restroom routines, transitions, outside play, lunch and nap. Staff records are located at a central location and will be reviewed at a later date. Emergency contact information for staff is maintained in each classroom on site. Children's records were reviewed. A walking tour of the inside and outside facilities was conducted. Annual inspections, required postings and monthly emergency evacuation drill documentations were reviewed. One violations related to age appropriate activities was observed and may be viewed on the violation notice.

Description: Based on observation and interview the center failed to ensure that activities are age appropriate. Evidence: 1 - At 10:22am 14 children in classroom 2 were all expected to participate in a pencil and paper activity where they were to trace their name, copy their name or independently print their name. At 10:45 there were still children sitting at the tables being encouraged to print their name. The children present for care ranged in age from 3 years to 5 years of age. There were three children at the table with child 1. 2 - A three year old child got up from his chair and began to cry out that he did not want to write his name. The child stomped his feet, and said no between crying. Staff 1 stated to the child "you are almost done, almost finished, come on you can do it." 3 - The child continued to cry as the staff guided him back into his seat and used hand over hand assistance to make the child complete the activity. a. The teacher was sitting on the child's left. She reached across the front of the child and placed her right hand on top of his right hand. The child was looking straight ahead not down at the paper. However, when instructed to look at the paper in front of him the child moved his head in a downward motion, but staff 1's forearm and hand made it impossible to see what was being printed. The child looked up and away from the activity three times. 4 - Staff 1 confirmed that child 1 was crying and stated that he did not want to complete the task.

Plan of Correction: Staff will be retrained in age appropriate structures for skill building.
