22VAC40-191 Background Checks (22VAC40-191)

On 3/21/16 a self-reported incident was received and initiated by the licensing office. The self-report investigated on-site, along with a mandated monitoring visit on 3/29/16 beginning at 11:45am. There is one violation, regarding supervision on the playground, that has been cited with the self-report. Any other violations cited are from the mandated monitoring visit. There were 140 children present today with 22 teachers. Children were observed eating lunch, washing hands, playing on the playground, and napping. Classrooms have projects displayed such as learning about senses, learning letters, making spring-themed flower pots, and learning time with clocks. Eight staff records, 6 children's records, and 7 medications with authorizations were reviewed. This inspection, including the exit interview, ended at 3pm. Standard #: 22VAC40-185-70-A
Description: Based on record review, 2 staff records (#3 and #8) do not have complete required documentation in their files. Evidence: Staff #2 is missing documentation of their highest level of education qualifying them for their job title. Staff #8 is missing documentation of two references prior to their start date (10/27/15).

Plan of Correction: Staff #3 will bring in education documentation immediately. We will call references again for staff #8 and place documentation in their file.

Standard #: 22VAC40-185-280-G
Description: Based on direct observation, a substitute container used for cleaning did not have a label indicating it's contents. Evidence: A substitute bottle being used in the KG class containing a solution of bleach water did not have a label.

Plan of Correction: The bottle was labeled during the inspection. We will remind staff to label substitute bottles.

Standard #: 22VAC40-185-340-F
Description: Based on discussion, written statements provided, and a review of documentation, three staff (#9,10 and 11) did not maintain sight and sound supervision of children when outside on the playground 3/18/16. Evidence: After a review of video footage by administration, it was determined that 3 toddlers left their class on the Toddler playground by opening the gate latch to the Beginner's playground, unnoticed by their three teachers. The children were playing unsupervised, out of sight supervision, for 4 minutes and 30 seconds before another staff in the adjoining School Age playground saw the children playing unsupervised. The children were then immediately brought back to the Toddler playground with their class. At all times the toddler children remained inside the main fenced area, on the school property.

Plan of Correction: We had maintenance raise the locks on the gate temporarily and are replacing them with permanent newer locks that children cannot open. All staff have been retrained on playground safety policies and supervision .
