Comments:
Today's visit was a routine monitoring inspection. The inspection began at 10:00am and concluded at 1:45pm.There were a total of thirty-two children present with seven direct care staff and the center's program Administrator was also present during the inspection. During the time of the inspection three out of ten staff records were reviewed, four out of sixty-two children's records were reviewed, school age children were observed in care as it was spring break week, outdoor play was observed, all areas of the physical plant were viewed, two medications were observed present at the center , lunch time was observed, nap time was observed, emergency supplies were viewed on all three levels of the building, and some policies and procedures were reviewed. Standard #: 22VAC40-185-510-C
Description: Repeat Violation-Based on the inspector's review of a total of two medications present in the center's medication lock box, the center failed to ensure a prescription long term medication had authorization from the parent as required by the center's medication policy. Evidence: The inspector observed prescription inhaler and reviewed the medication authorization. The authorization form did not include parent authorization. The medication authorization form did include a physician's authorization indicating long term use. There was no evidence that the medication had been administered to the child.

Plan of Correction: The Program Director will have the parent complete the top portion of the medication authorization for.
