During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
- Technical Assistance:

* Discussion of Code of Virginia 22.1-289.057.B - Provider will send email from Fairfax Water to Licensing Inspector by 06/25/2025, to establish compliance. Provider has been in contact with this company in regard to kits and testing.
* Discussion that for programs subject to national fingerprint background checks pursuant to ? 22.1-289.035 of the Code of Virginia, VDOE will be unable to continue to cover the cost of national fingerprint background checks beyond December 31, 2025. Effective January 1, 2026, child day programs and family day systems will be responsible for covering the cost of obtaining fingerprint-based background checks. Child day programs and family day systems are encouraged to plan for this change. When preparing for operating costs beyond December 2025, fingerprint fees for employees, applicants, agents, caregivers, approved providers in a family day system, and household members will be $59.00 per individual.
* Discussion to send in through the provider portal a completed FDH Renewal Application as soon as possible and before your expiration date of 06/29/2025. Failure to do so may result in the closure of your license.

An unannounced, on-site renewal inspection was initiated and completed on 06/11/2025. The on-site inspection began at 9:30 am and ended at 11 am. The inspector reviewed compliance in the areas listed above. There were (3) children present and (2) caregivers; the point total was (10). The inspector reviewed (3) children?s records, (2) caregivers records, (1) household member record on-site on 06/11/2025.
This inspection included: document review (i.e. Files, injury logs (none), policies and procedures, emergency drill logs); tour of the facility; interviews; and observations.
Information gathered during the inspection determined (1) non-compliance with applicable standard. This violation was documented on the violation notice issued to the program.

Standard #: 8VAC20-800-800-B Description: The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.
The last documented review of the emergency plan was conducted 23 months ago.Plan of Correction: The Family Day Home's Emergency Preparedness and Response Plan was reviewed and signed off on today during the inspection.
