During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated and completed on 6/5/2025. The on-site inspection began at 9:05am and ended at 10:05am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers. The inspector reviewed 3 children?s records and 2 caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/18/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-800-830-B Description: Shelter-in-place procedures shall be practiced a minimum of twice per year.
Provider did not conduct any shelter-in-place drills from May 2024-May 2025.Plan of Correction: Already conducted one shelter-in-place drill. Wrote down the requirements for the required drills.
Standard #: 8VAC20-800-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
The record for Child B, enrolled for ten months, was missing one name, address and telephone number for designated persons to contact in case of an emergency if the parent cannot be reached.Plan of Correction: Parent made corrections to missing information.
Standard #: 8VAC20-800-280 Description: Potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents and disinfectants shall be stored away from food in areas inaccessible to children.
The was a tub of disinfectant wipes Labeled ?Keep out of reach of children? on a low shelf on the diaper changing table, accessible to children.Plan of Correction: Corrected during the inspection.
Standard #: 8VAC20-800-530-A Description: Cribs when being used by infants shall have a tight-fitting bottom cover
Child A (14 months) was observed in a crib that had no covering on the mattress.Plan of Correction: Sheet was placed on the crib same day.
