Comments:
An unannounced monitoring inspection was conducted from 10:20 a.m. - 1:25 p.m. There were forty-five children in care with six teachers working directly with them. There was another teacher available to assist as needed. Three children's records were reviewed and the record for one teacher was reviewed. The younger children were observed discussing appropriate classroom behavior, engaged in working at different work stations the teachers had prepared for them, during self-help skills putting on their shoes and coats to go outside and laughing and playing ball outside. The older children were working with the rectangle box, making cloth mittens, putting geographical puzzles together while discussing different countries, playing group activity games, during hand washing, toileting, snack, singing songs from memory, singing grace before lunch and during nap. Standard #: 22VAC40-185-40-D
Description: Based on inspection the facility failed to have the most recent license posted in a place conspicuous to the public .

Evidence:

1. On 03/31/2016 the licensing inspector reviewed the posted license and it was dated December 5, 2013 through December 4, 2015. The most current license was issued for December 5, 2015 through December 4, 2017.

2. On 03/31/2016 staff #1 reviewed the license posted and confirmed the most recent license was not posted.

Plan of Correction: I immediately found the new license and posted it. It had been filed.

Standard #: 22VAC40-185-60-A
Description: Based on a review of three children's records the facility failed to have the addresses for two designated people for each of two children in an emergency. Evidence: 1. On 03/31/2016 the licensing inspector reviewed the record for child #1 and child #2 and there were no addresses for the emergency contacts. 2. On 03/31/2016 staff #1 reviewed the records and emergency cards for child #1 and child #2 and there were no addresses for the the emergency contacts.

Plan of Correction: I will be able to get the addresses for one child today and the other child by next week. I will check our records to ensure we have addresses for emergency contacts as required.
