An unannounced monitoring inspection was done today March 30, 2016 beginning at 8:40 AM and ending at 2:30 PM. There were fifty-two children in care today. Some school age children were present due to Spring break for the public schools. The children were observed during their worship time and outside time. Morning snack and lunch were also observed. The children played outside on riding toys and in the rock box area. They were also playing at the table with play dough. The mulch was tilled and raked today during the inspection. There were no medications on site today. The inspector also reviewed a sample of staff and children's files. This sample included newly hired staff and enrolled children. The violation was discussed with the director today as well as corrective action. If you have any further questions, please contact the inspector at 540-309-2835.
Standard #: 22VAC40-185-150-B Description: Based on observation the center did not obtain all of the required information for immunizations obtained for the children enrolled. 1. During the inspection the LI reviewed the files of five children and child # 4 had a copy of immunizations but there was no documentation that they were adequate. There was also no signature of a physician/his designee. Plan of Correction: The director indicated she would have the parent obtain a statement and signature from the physicians office. The director will notify the parent today.
